Care Street Home Health Care LLC is a division of Ocean Healthcare, a network of New Jersey-based healthcare centers and services founded in 1973 by Melvin and Debbie Feigenbaum. Melvin and Debbie both came from warm, close-knit backgrounds that emphasize the centrality of family. Kate Jenkins, Administrator
Kate Jenkins is the Administrator of Care Street Home Care Agency servicing Central and Northern NJ as well as the Jersey Shore. Kate has made it a primary goal to provide clients with an outstanding care experience through education, enhanced Coordination of Care, support, advocacy, and by emphasizing an interdisciplinary holistic team approach. Kate’s 15 years of experience in the Sub-Acute, LTC, Home Care, Skilled Nursing, Care Management and Consulting render her ideally suitable for this position and up to the challenge.
Kate’s mission at Care Street is to implement and coordinate Care Management Models of Disease Management; Alzheimer’s/Dementia, Diabetes, Arthritis, Cardiac, Stroke, Parkinson’s, Pulmonary Dysfunction, Fall Prevention, Orthopedics, Generalized Pain, Oncology and Hospice, Care Management on the Certified, RN and SW levels, all through upholding the CAHC and Best Practice guidelines for Home Care patients, focused on Patient Centered Care Experiences.
Dr. Shelly Chinkes, DPM, Administrator
Dr. Shelly Chinkes, DPM, Care Street’s able administrator has more than 30 years of clinical experience in private practice and Skilled Nursing settings, with specific experience in clinical case management in Gerontology.
Dr. Shelly, as he is fondly known, has served as an Alzheimer’s Support Group Facilitator in Mercer County and is a Certified Dementia Instructor. Knowledgeable, compassionate, and unusually devoted, his guidance is crucial in helping families understand their options and render decisions for their loved ones’ care plan. Dr. Shelly’s extensive experience, sincere and pleasant demeanor, and professional affiliations have made him a vital asset to Care Street.
